Please post the contents of those files: /lib/udev/rules.d/11-dm-lvm.rules and /lib/udev/rules.d/84-nm-drivers.rules.

You may try commenting out the following line:

Or you can try updating to sys-fs/lvm2-2.03.06 or greater.
