VIDEO_CARDS="Intel"
, tried both modesetting and old DDX driver.

I thought it might be a problem caused by -O3, but tried using xorg-server, glibc and mesa from official gentoo binhost where all packages are compiled with:
Code:
CFLAGS="-O2 -pipe -march=x86-64 -mtune=generic"
and got the same result. Of course I used same CFLAGS on binhost.

Why do you add -flto into CFLAGS on an old machine?

Of course the same result is expected, as the intel driver is from xorg-drivers package which would have been affected by your CFLAGS.

Try recompile whole system with just -march=core2 -O2 -pipe please.

Why not? I didn' t saw any caution for old machines about it.

Nah, I pulled xorg-drivers from official binhost too.

It will lengthen your compile time even more. And who knows if it is the flag responsible for this issue.

I've been using my main system as binary host so I didn't care about compile time, and packages from official binhost are built without LTO and they don't work either.
To make this clear, I made
Code:
emerge --deselect x11-base/xorg-server media-libs/mesa sys-lib/glibc
, then
Code:
emerge --depclean
, then installed packages from official binhost by
Code:
emerge -avg x11-base/xorg-server media-libs/mesa sys-lib/glibc
so all dependencies such as xorg-driver was pulled from official binhost too.

What instruction is causing the fault, and in what library is it located? What package owns that library?

Both libgbm.so and crocus_dri.so are owned by media-libs/mesa, I don't know how to check what instruction is causing the fault, ran
Code:
objdump --disassemble=xorg_backtrace | grep ad2
and it showed
Code:
186ad2 test %eax, %eax
but I'm rather wrong.

The only place I see ad2 is in the tail of the address in the Xorg binary, but I suspect that is part of its crash report handler. You could disassemble in crocus looking for suspicious instructions (particularly around ..446), or you could trap the signal when it occurs, at which point $rip would point directly to the faulting instruction.

How this can be done?

Ok, I think that illegal instruction might be
Code:

movhps 0xc(%rcx,%rax,%1),%xmm0
