How can I force a rebuild of all installed packages. I've made some changes to the C* flags in /etc/make.conf and would like that to now apply to everything on my system.

Try taking a peek at the -e/--emptytree flag. From emerge --help:

--emptytree (-e short option)
Virtually tweaks the tree of installed packages to only contain
glibc, this is great to use together with --pretend. This makes
it possible for developers to get a complete overview of the
complete dependency tree of a certain package.

For example, if you want to recompile all the system dependencies, try:

emerge -e system -p

Just remember to take the that pretend flag out, from my experience, it seems that there is a limit to the total number of packages you can emerge. For example,

# emerge -e -p system | wc -l
97
# emerge -e -p world | wc -l
97

The world one should be much bigger, my advice is to compile the system first, and then try compiling with -e for specifics like xfree.

Make sure you have something else to do before you start it.
