OPC failed with Plextor 708A and k3b while writing DVD

Well, things were running well a little while ago, but somehow, something changed and now it's broken again. Whenever I try and burn a DVD, it comes with OPC failed and an input/output error. I've tried many solutions, such as commenting out the OPC kill line in the growisofs command, but it burns super slowly and doesn't work at all. Incremental write doesn't work either. I'm on the latest version of udev with 2.6.11-r5. Can anyone help?
