I have an ATI SB400 Modem (some sort of soft modem, I presume).

The kernel has a driver, and it recognizes the modem during boot:

  #0: ATI IXP rev 1 with unknown codec at 0xb0003400, irq 20
  #1: ATI IXP Modem rev 1 at 0xb0003800, irq 20


Where do I go from here in using this modem (in particular,
where do I find the modem in /dev so I can access it - I don't
see it in any of the places I would expect)?

You have to emerge the slmodem driver for this modem. The modem will show up as /dev/ttySL0 after the driver is installed.
