updating ALL installed packages

How do I update all installed packages? emerge --update system and emerge --update world just updates the packages listed in the system and world variables, but these are not all...

As far as I know, that should be everything unless you installed something without Portage. The list of packages for world are stored in /var/cache/edb/world
