I've got the Intellimouse Optical, and I've never had any of the problems you're describing with it skipping stages or scrolling sideways. Sorry if you've already tried something similar, but this is the mouse section of my XF86Config:

xmodmap -e "pointer = 1 2 3 6 7 4 5"
should let you use the two side buttons and the wheel.

yes, I've tried that method and that's what I'm using now, it still skips stages AND scrolls sideways simultaneously.. with imwheel it scrolls sideways but doesn't skip stages.. And I haven't had this skipping stages problem on over 9 different distros both unix and linux alike which is very strange.

Maybe someone can post a working XF86Config with which the Microsot Intellimouse works?

WOHAY, I fixed the problem by getting rid of SendCoreEvents!
