Some packages like mplayer have optional USE flags for processor extensions like sse2, 3dnow, etc. If these are enabled and subsequently the code was run on a machine that didn't support this extension, will bad things happen, or will it simply ignore the optimizations?

Well, if you consider failing compiles bad, then yes bad things can happen. You might see binaries failing with illegal instruction error messages too.
