DEC VRC15 Monitor problems (Nvidia)

I am having trouble with seting up this monitor it is an old monitor that I can not find the refresh rates on so I am having to guess. It is a 15 inch monitor and onthe back label it says 50 - 60 Hz. Any one have any suggestions. Everytime I start a new session it is small and off centered. Also when opening a window it is larger than the monitor and the left side of the window overlaps to the right side of the monitor.

Also I am trying to set up my NVIDIA GF 4 for 3D. I tried running glxgears and it gave me an error.

I did the (opengl-update nvidia) and did not receive any errors. I changed the xf86config to enable glx and commented out dri. Any help would be appreciated.
