This is a recent issue that's come up.

A few days ago I was fooling with my XF86Config-4 file, setting up Xinerama. That required many restarts of X, in which I noticed the mouse stopped acting how it should. I had a different mouse theme, Blue-Cursor (from the thread in the Tips forum), and it defaulted to the regular X cursor. I was also using IMWheel, as I have an Internet Explorer mouse, with 7 buttons. The two thumb buttons have stopped going back and forward in Firebird, where they worked several days ago.

I have no idea what the problem is, and I'd love to figure it out, but I'm at a dead end as to what it could be. Any help is greatly appreciated
