I'm using currently x11-base/xorg-X11 should I switch it to x11-base/xorg-server ?
I've noticed that Gentoo documentation prefer xorg-server over X11

Do I need to un-install x11 to switch it to xorg-server?
I want the transition to go smooth as this is a remote machine.

I just noticed my system has both installed:
x11-base/xorg-x11-7.4-r1
and
x11-base/xorg-server-1.11.2-r2

How to find out which one is in use?

by reading through the description.
xorg-x11 is a (legacy) meta-package, while xorg-server only pulls in the required parts. downside of using plain xorg-server, is that you need to install some tools and fonts separately.

just be sure that ALSA_CARDS, VIDEO_CARDS and INPUT_DEVICES are correctly setup in your make.conf

Good advice, in my case it seems to me I've migrated to xrog-server but forgot to unmerge xorg-X11
Nothing is listed for dependency xorg-X11

Review packages that will be removed, then:

Thanks, it is done. As I suspected I forgot to remove the x11 after upgrade to xorg-server.

What are you talking about ? xorg-X11 is a meta-package which installs all of X11 infrastructure, including xorg-server (which is just a server part of it).
Some people do not install xorg-X11 if they want a more compact installation and install just xorg-server with its dependences. Removing xorg-X11 does not remove anything of substance, just meta package info
